Information about Memorial Plaques

Please fill out and return this form to the Museum in order to purchase a memorial plaque.

Cost:  $25 Memorializes One Individual

*Contributions to the project can be made anonymously

All Contributors will Receive the Following
:

· Plaque with memorialized name, birth, death, and service dates, and service branch.

· The Memorial Wall will be a part of the exhibit from May 2013- May 2015.
· At the close of the exhibit, the wall will be displayed permanently in another part of the museum.
· Invitation to exhibit opening. Names of all memorialized will be announced· Photographs of individuals will be included on the museum website during the opening week (if provided).

Payment can be made by check, credit card (in person or over the phone), or cash (in person). Please write WWII memorial in memo line of checks. Please do not mail cash.
